An underground economy for unlicensed slimming drugs is being driven by TikTok fitness influencers and sellers on messaging platforms, based on recent findings.

Separately, a robust trade exists on Telegram, where communities with thousands of members share “before and after” photos, administration instructions and claims of next-day British shipping. Sellers market the substance there before directing buyers to other platforms for transactions.

Retatrutide is an investigational injection developed by American pharmaceutical company Eli Lilly. The treatment affects three gut hormones – GLP-1, GIP and pancreatic hormone. Preliminary studies indicate it could assist individuals lose up to a quarter of their body weight. Referred to as the “triple G”, the drug is being hailed online as the next Ozempic.

However, physicians and pharmacists have warned that the sale of what is still an unapproved drug in the United Kingdom is worrisome and could lead to users injecting products of unknown origin.
Several professionals said the demand for the experimental drug could be connected to the rising price of approved weight loss products such as Mounjaro.

The substance is not a restricted substance, but promotion and distribution can still breach medicines, advertising, and consumer laws, potentially leading to penalties, legal action or imprisonment.

A spokesperson for Telegram said it actively monitored harmful material on its platform including material that encouraged the “misuse of controlled medications”. The company noted that it eliminated “millions of pieces of harmful content each day”.

“People are injecting substances they cannot confirm. Even if the research behind retatrutide is sound, these digital products may not include what they advertise,” said a pharmacy expert, the head of pharmacy at Chemist4U.
Dr Crystal Wyllie of Asda Online Doctor linked the surge in demand to current price rises for approved weight-loss medications such as Mounjaro. “With costs increasing, individuals are using social media. But counterfeit treatments are extremely dangerous. You can’t be certain what’s inside or whether the conditions they were made in was clean,” she explained.
Her staff has already encountered patients injured by personal administration from unverified suppliers. “We recognize the disappointment, but buying from messaging apps or WhatsApp is taking chances with your health,” said Wyllie. “These materials could be contaminated or improperly measured.”
Manufacturer Statement
A spokesperson from Eli Lilly said the substance was in third stage clinical trials and had not been reviewed or authorized by any regulatory agency globally. “Therefore at this point no one can market [it] for human use,” they said, adding that any item “falsely representing itself as a company experimental substance not yet approved … may expose users to potentially significant medical dangers”.
